Physician Assistant or Nurse Practitioner 4 Day Float
About The Position
Low-T, a leader in hormone replacement therapy and targeted wellness, is seeking a Full-Time 4 day Float Mid-Level Provider (NP or PA) . This is an exciting opportunity for providers who value patient-centered care, work-life balance, and a collaborative clinic environment . Key Responsibilities Provide hormone replacement, weight management, and targeted wellness care Conduct patient assessments, create individualized treatment plans, and follow-up care Educate patients on wellness, treatment protocols, and lifestyle optimization Collaborate with Center Director, Medical Director, and clinic staff Maintain accurate documentation and compliance with clinic protocols Qualifications Active Indiana NP or PA license in good standing Current DEA license or ability to obtain Strong communication and clinical decision-making skills Schedule 32 clinical hours per week Clinical shifts: Tuesday, Thursday, Friday, and Saturday's Clinic hours: Mon 8–5 | Tues 8–7 | Wed 8–1 | Thurs 7–5 | Fri 8–5 | Sat 8–12 Compensation & Benefits Salary: $110,000 PTO, medical/dental/vision, short- and long-term disability Malpractice insurance covered Scrubs provided CME allowance + CME PTO Licensure renewal reimbursement 401(k) with employer match Paid training: 2 weeks (10 consecutive business days) Apply today to join a team dedicated to improving patient health and well ness .
Requirements
- Active Indiana NP or PA license in good standing
- Current DEA license or ability to obtain
- Strong communication and clinical decision-making skills
