About The Position

This position offers the opportunity to work in a dynamic, team-based environment focused on optimizing care for patients with complex valve pathologies. The APC will play a central role in coordinating care, managing clinic visits, supporting inpatient consults, and facilitating multidisciplinary collaboration. While direct provider involvement in valve pathology is minimal, the APC will be instrumental in ensuring continuity and quality of care.

Requirements

  • Graduate of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program
  • Current California NP or PA license
  • DEA Controlled Substance Registration Certificate (CSRC)
  • Current BLS certification
  • Current ACLS certification
  • Additional certifications as required by department
  • Prior experience in cardiology, heart failure, structural heart disease, or related specialty required

Responsibilities

  • Provide patient care in both outpatient and inpatient settings, including initial assessments, follow-up visits, and hospital-based consults
  • Coordinate care across specialties including Advanced Heart Failure, Interventional Cardiology, and Cardiothoracic Surgery
  • Assist with pre- and post-procedural planning and education
  • Conduct telehealth visits and manage patient communications via the Epic platform
  • Participate in multidisciplinary valve conferences and contribute to care planning
  • Educate patients and families on valve disease, treatment options, and care pathways
  • Support quality improvement and patient experience initiatives
