About The Position

Corewell Health Southwestern Medical Clinic Primary Care is looking for a dynamic, experienced Physician Assistant or Nurse Practitioner to join their team of physicians and APP's at our busy Stevensville location. This is a full-time position which is considered 36 patient contact hours. The current team consists of 8 provides including 5 physicians and 3 APPs. The office is open Monday - Friday from 8:00 am to 5:00 pm. Call is 1 in 7, 1 week day per rotation. Providers typically see 18 -20 patients per day including; new patient visits, acute care visits, follow-up, new problem, physical exams, pap and pelvic exams, and some pediatric patients. The office uses EPIC as our EHR and Fluency dictation is avilable. Responsible for managing patients and coordinating the services provided by multiple disciplines within and outside the health system. Maintains a collaborative practice agreement with specific physician(s). Exercises independent judgment in the assessment, diagnosis and initiation of selected protocols/procedures. Provides educational support for patients/significant others, health team members and the community and coordinates QA/QI studies to measure outcomes of care. Responsibilities

  • Functions in the role of advanced practitioner to coordinate comprehensive care delivery. Writes orders/prescriptions for standard formulary medications, laboratory, radiographic studies and consultations, coordinating the scheduling of admissions, diagnostic evaluations, follow-up exams, laboratory tests, referrals to home care, social services, and other community agencies, etc., in order to ensure continuity of care and coordination of services.
  • Obtains histories and performs physical exams.
  • Assesses the patients/significant other, health care team, school and other community educational needs and develops, implements, evaluates and revises teaching strategies/educational programs related to area of specialty.
  • Acts as a patient advocate, crossing multidisciplinary lines to meet patient needs.
  • Communicates, collaborates, networks with and acts as a consultant to other members of the health care team within and outside Spectrum Health. Acts as a consult in area of expertise.
  • Performs and assists with designated procedures.
  • Documents in the medical record and dictates information as appropriate.
  • Provides input into the strategic planning, implementation and evaluation of service/program. Measures the outcomes of care and evaluate services across the continuum of inpatient to outpatient services. Makes recommendations to improve the quality of patient care provided and utilizes the scientific method of research clinical problems.
