Nationally certified Advanced Practice Provider who provides primary and/or specialty care in a variety of healthcare settings. Functions in collaboration with the health care team in accordance with certification and licensure. Practice emphasizes health promotion, disease prevention and the diagnosis and treatment of acute and chronic health problems. Responsible for dissemination of knowledge through instructing, leading and developing educational programs. Cleveland Clinic's Heart, Vascular and Thoracic Institute's world-renowned Heart Transplantation team has a full- time opening for an experienced APP. The ideal candidate will be energetic, motivated, team oriented, possess excellent verbal and written communication skills, and provide exceptional care to patients. The patient population served by the advanced practice provider will be patients listed for heart transplantation, post LVADS, and post heart transplantation recipients. This provider will manage patients independently, primarily in the outpatient setting. This patient population requires complex monitoring and therapies, high intensity interventions, and continuous vigilance within the range of high-quality care while adhering to UNOS and CMS regulations. Transplant experience is preferred. Functions in collaboration with a physician according to a standard care arrangement and specialty certification. Anticipated Schedule: Days, 5-8's (40 hours a week)
