About The Position
The Field Nurse Practitioner (NP)/ Physician Assistant (PA-C) position is a full-time role within a reputable, value-based in-home provider organization in Herkimer, NY. The primary focus of this role is to enhance the quality of life for patients by delivering comprehensive care directly in their homes. The position involves visiting 3-6 patients daily, providing primary, urgent, and palliative care to the adult geriatric population, while collaborating with an interdisciplinary team to address the medical, social, and emotional needs of patients.
