( OPO Health is licensed in over 15 counties. )

Requirements

  • Active and unencumbered California Physical Therapist (PT) license
  • Valid California driver’s license and proof of auto insurance
  • Minimum of one (1+) year of relevant experience
  • Current CPR certification

Responsibilities

  • Deliver physical therapy services in accordance with the physician’s written plan of care
  • Assess and evaluate patients’ therapeutic, rehabilitative, and functional status
  • Participate in the development and adjustment of the overall plan of care
  • Assess muscle strength, mobility, gait, range of motion (ROM), and rehabilitation potential
  • Administer and direct physical therapy treatments
  • Instruct patients and caregivers in the use and maintenance of therapeutic equipment or appliances
  • Identify and prioritize physical therapy needs
  • Report patient responses to treatment or changes in condition to the physician
  • Initiate physical therapy programs and guide family members, caregivers, or other personnel in specific phases of therapy to support patient progress
  • Provide education to patients and caregivers regarding therapy goals and techniques, participating in case conferences as necessary
  • Identify patient and family/caregiver needs for additional home health services and make appropriate referrals
  • Prepare and submit clinical documentation and progress reports based on patients’ goal attainment
  • Contribute to discharge planning to ensure a smooth transition of care
  • Maintain accurate and timely clinical notes and provide updates to the primary physician as needed
  • Communicate effectively with physicians regarding patients' conditions, needs, and any necessary adjustments to care plans, obtaining orders as required
