( OPO Health is licensed in over 15 counties. )
Requirements
- Active and unencumbered California Physical Therapist (PT) license
- Valid California driver’s license and proof of auto insurance
- Minimum of one (1+) year of relevant experience
- Current CPR certification
Responsibilities
- Deliver physical therapy services in accordance with the physician’s written plan of care
- Assess and evaluate patients’ therapeutic, rehabilitative, and functional status
- Participate in the development and adjustment of the overall plan of care
- Assess muscle strength, mobility, gait, range of motion (ROM), and rehabilitation potential
- Administer and direct physical therapy treatments
- Instruct patients and caregivers in the use and maintenance of therapeutic equipment or appliances
- Identify and prioritize physical therapy needs
- Report patient responses to treatment or changes in condition to the physician
- Initiate physical therapy programs and guide family members, caregivers, or other personnel in specific phases of therapy to support patient progress
- Provide education to patients and caregivers regarding therapy goals and techniques, participating in case conferences as necessary
- Identify patient and family/caregiver needs for additional home health services and make appropriate referrals
- Prepare and submit clinical documentation and progress reports based on patients’ goal attainment
- Contribute to discharge planning to ensure a smooth transition of care
- Maintain accurate and timely clinical notes and provide updates to the primary physician as needed
- Communicate effectively with physicians regarding patients' conditions, needs, and any necessary adjustments to care plans, obtaining orders as required
