The position available is for a Physical Therapist (PT) in Home Health, with employment status options of Full-Time, Part-Time, or Per Diem. The location for the caseload is tailored to the preferred location of the candidate, as OPO Health is licensed in over 15 counties.

Requirements

  • Active and unencumbered California Physical Therapist (PT) license.
  • Valid California driver’s license and proof of auto insurance.
  • Minimum of one (1+) year of relevant experience.
  • Current CPR certification.

Responsibilities

  • Deliver physical therapy services in accordance with the physician’s written plan of care.
  • Assess and evaluate patients’ therapeutic, rehabilitative, and functional status.
  • Participate in the development and adjustment of the overall plan of care.
  • Assess muscle strength, mobility, gait, range of motion (ROM), and rehabilitation potential.
  • Administer and direct physical therapy treatments.
  • Instruct patients and caregivers in the use and maintenance of therapeutic equipment or appliances.
  • Identify and prioritize physical therapy needs.
  • Report patient responses to treatment or changes in condition to the physician.
  • Initiate physical therapy programs and guide family members, caregivers, or other personnel in specific phases of therapy to support patient progress.
  • Provide education to patients and caregivers regarding therapy goals and techniques, participating in case conferences as necessary.
  • Identify patient and family/caregiver needs for additional home health services and make appropriate referrals.
  • Prepare and submit clinical documentation and progress reports based on patients’ goal attainment.
  • Contribute to discharge planning to ensure a smooth transition of care.
  • Maintain accurate and timely clinical notes and provide updates to the primary physician as needed.
  • Communicate effectively with physicians regarding patients' conditions, needs, and any necessary adjustments to care plans, obtaining orders as required.
