About The Position

PHYSICAL THERAPY ASSISTANT Under supervision of a licensed physical therapist and the rehabilitation supervisor, implements medically prescribed physical therapy programs (i.e. modalities, exercise, teaching, etc.) directed toward relieving pain and/or restoring or maintaining physical function. SPECIFIC DUTIES AND FUNCTIONS: Works in conjunction with the interdisciplinary team (IDT) to coordinate rehabilitation services for PACE Southeast Michigan (PACE SEMI) participants. Assists in coordination of 24-hour care needs. Provides treatments and advances plan of care; coordinates discharge or level-of-care planning, collaborating with other health care providers. Documents all care given to participants in accordance with departmental and professional standards using the Electronic Medical Record (EMR) and other tools as indicated in a timely fashion. Actively participates in professional associations, educational programs, clinical development, and in-service education. May delegate, direct, and/or supervise the activities of support staff (Rehab Technicians, Participant Care Technicians) providing necessary communication and guidance. Assist Therapists with initial and periodic assessments including in-home and facility based as needed, prior to care conferences. Communicate any participant changes to team members and document accordingly. Assist with management of Durable Medical Equipment (DME) within scope of practice area. Performs other related duties as determined by Rehabilitation Manager or other Leaders which may include components of program development, quality assurance, or other areas/projects. Maintains appropriate clinical productivity standards. Maintains an awareness of participant safety issues within the facility and in use of equipment, making recommendations as needed for improvement.

Requirements

  • Associates degree as a Physical Therapist Assistant from a school accredited by the APTA
  • Current state of MI license
  • One year (1) minimum experience with a frail or elderly population
  • Physical ability to constantly exercise, lift patients, push wheelchairs, and move equipment
  • Current CPR for Healthcare Providers
