Job Summary Provide physician prescribed physical therapy under a plan of care established by the PT Improve or minimize residual physical impairments of the patient Participate with all other health care personnel in patient care planning Perform all skilled procedures as ordered by the physician and according to the plan of care established by the PT Consult with PT regarding change in treatment Participate in educating the patient and the patient's family Participate in in-service programs as assigned Attend all patient care conferences as scheduled Prepare medical records, progress notes and update care plans for each patient visit in a timely manner as per policy
