About The Position

Position Summary: Under the direction of a NYS licensed Physical Therapist, the Physical Therapy Assistant (PTA) provides and documents physician-ordered occupational therapy services. The PTA provides superior customer service by modeling the Unity Brand Promise and Core Values. Status: Per Diem Department: PRLC Rehabilitation Location: Park Ridge Living Center Schedule: Varies 6-hr shifts Key Responsibilities: Within standards of practice established for a PTA, completes and documents (within agency standards) related to patient care, including visit notes, renewal orders, and verbal orders. Includes progress or lack of goal achievement in all charting. Implements treatment programs individualized to meet patient needs and discharge plans: issues and provides instruction in the use of adaptive equipment. Confers with therapist directly, physician, and/or interdisciplinary team members regarding patient response to therapy and any concerns. Provides PT gait and ambulation training. Instructs in the use of assistive devices. Provides instruction in safe transfer using appropriate body mechanics and equipment. Participates in discharge planning and provides timely and adequate patient and family teaching to facilitate the discharge plan. Communicates regularly with the Physical Therapist who oversees the patient’s plan of care. Coordinates patient schedule with team members, readily and effectively adjusts schedule to meet changes in census, patient needs. Maintains professional growth through active participation in continuing education programs, departmental in-services, etc. Communicates pertinent information to patient and family members, including goals, procedures, progress, etc., as appropriate. Displays the capacity for motivating others. Recognizes the effects of his/her own non-verbal behaviors upon others. Correctly interprets and responds to the non-verbal behavior of others HHCS Only: Evaluates home environment for changes needed to increase the patient’s independence. Recommends equipment and assistive devices (ramps, bathroom aides, other adaptive equipment) as appropriate. HHCS Only: Maintains required agency visit productivity.

Requirements

  • Associate's Degree from an accredited Physical Therapy Assistant program.
  • Minimum of 2 years post-graduate experience as a PTA in an acute care setting and/or an acute rehabilitation facility.
  • Successful completion of all fieldwork requirements and eligible to sit for the PTA or OTA exam.
  • Reads, writes, and speaks English.
  • Must demonstrate computer proficiency.
  • Excellent interpersonal and communication skills.
  • Current NYS PT Assistant licensure required.
