About The Position
The Physical Therapy Assistant at Shadelands Post Acute in Walnut Creek, CA, plays a crucial role in the rehabilitation and recovery of patients under the direction of a licensed Physical Therapist. This full-time position involves treating patients as per the treatment plans established by the Physical Therapist, ensuring that all treatment notes and progress reports are accurately recorded in compliance with the Physical Therapy Board's regulations. The assistant will actively participate in patient care and rehabilitation conferences, contributing to the overall treatment strategy and patient outcomes. In addition to direct patient care, the Physical Therapy Assistant is responsible for maintaining the treatment area, ensuring it is clean and organized. This includes assisting with the cleaning and maintenance of equipment and reporting any issues that may arise with departmental tools. The assistant will also play a vital role in educating patients' families and nursing staff about the maintenance programs and caregiver training necessary for a successful discharge from therapy services. Communication is key in this role; the assistant must effectively relay patient progress and any concerns to supervisors and other healthcare team members. Participation in in-service training programs for staff is also expected, fostering a collaborative environment that enhances patient care.
