Neighborhood Healthcare PACE is a managed medical plan built around surrounding participants with a team of physicians, nurses, social workers, therapists and care coordinators to help them maintain good health and a good quality of life. Our goal is to keep our seniors happy and healthy at home surrounded by their family and community. As a private, non-profit 501(C) (3) community health organization, we serve over 500k medical, dental, and behavioral health visits from more than 100,000 people annually. With two PACE centers located in Riverside County, our PACE program is positioned to serve over 650 senior participants. The Physical Therapist Assistant works to support the mission and vision of Neighborhood Healthcare by supporting participants rehabilitative physical therapy (PT) treatment. This role will assist physical therapist with participant evaluations and treatment plans for program participants. In addition, this role will assist participants to remain independent and live safely in the community. This role/site is eligible for a $2,500 signing bonus! Schedule: M-F 7:30/8:00am-4:00/4:30pm. This position is fully onsite.

Requirements

  • Associate degree in physical therapy assisting required; an emphasis on geriatric populations is preferred
  • California Physical Therapy Assistant license in good standing is required
  • One year of physical therapy assistant experience working with the geriatric population preferred
  • Basic Life Support (BLS) through the American Heart Association certification required upon hire
  • Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
  • Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
  • Knowledgeable about and experience with principles and methods for treatment and rehabilitation of disabilities and functional impairments
  • Ability to use ingenuity and imagination in adapting modalities to individual needs
  • Knowledgeable and understanding of Older Americans Act, Americans with Disabilities Act, and Area Agencies on Aging
  • Ability to establish and maintain therapeutic relationships and to participate in a multi-cultural, multidisciplinary team. and strong interpersonal skills
  • Ability to successfully manage multiple tasks simultaneously
  • Excellent planning and organizational ability
  • Ability to work as part of a team as well as independently
  • Ability to work with highly confidential information in a professional and ethical manner
  • Ability to lift/carry 50 lbs/weight
  • Ability to stand for long periods of time

Responsibilities

  • Assists physical therapist implement PT programs for participants accessing day services, including therapeutic exercises, neuromuscular reeducation, rehabilitation services, functional activities, gait trainings, orthotics trainings, prosthesis, and trainings
  • Makes recommendations to the physical therapist to modify the program based on the patient population, as needed
  • Performs assessments of patients to determine abilities
  • Assists physical therapist implement individualized plans of care to decrease or eliminate disabilities, increase or maintain capabilities for independence, and develops functions to a maximum level
  • Ensures individual plans of care are followed accurately
  • Provides a variety of physical therapies aligned to participants plans of care, including functional ability tests, treatment to relieve pain, and develop or restore function
  • Assists participants to achieve and maintain maximum performance using physical means, such as exercise, heat, sound, water, light, or electricity
  • Guides patients and caregivers in the use of adaptive devices and therapeutic, creative, and self-care activities for improving function
  • Evaluates the effectiveness of treatments and makes recommendations to modify goals, as needed
  • Records and signs treatments in the patient's health record
  • Ensures services provided are in the clinic are conducive to restoring dignity and self-esteem to all patients
  • Assists physical therapists to ensure that required equipment and supplies for the physical therapy program are available and/or ordered, as needed
  • Keeps abreast of all regulations and standards to ensure compliance with orders or directives issued by duly constituted government and regulatory agencies (PACE, CMS, DHCS)
  • Participates in training programs for staff and caregivers
