What you will do: Promote/provide physical therapy and/or occupational therapy services in the home to assist the patient in achieving his/her optimal level of functioning. Evaluate and treat individuals with functional deficits secondary to neurologic, orthopedic and medical conditions. Teach patients/caregivers and help them to become independent with their treatments and self-management of their chronic illness(es) Experience the rewards of appreciation from being able to address the whole patient Coordinate an interdisciplinary plan of care with nursing/physical/occupational/speech therapists, medical social workers, aides and dieticians. Minimal weekend and holiday rotations

Requirements

  • Current NYS PT or OT license and registration
  • Minimum of one (1) year of experience as a PT or OT, preferably inpatient or homecare, is required
  • Valid Driver's License and Reliable Vehicle.

  • Home care experience preferred, but not required

  • Pay is based on experience, skills, and education.
  • Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
  • The pay range may also vary within the stated range based on location.
