About The Position

Mosaic Life Care is a health care system in northwest Missouri. This position evaluates, plans and implements treatment programs for the rehabilitation of patients, as directed by a provider, using approved treatment procedures. Provides patient/family education. Participates in family conferences. Responsible for all necessary documentation as required by accepted medical guidelines. This position works under the supervision of Manager of Therapies department and is employed by Mosaic Health System.

Requirements

• Bachelor's Degree - Graduate of accredited school of physical therapy is required.
• Licensed Physical Therapist in State of Missouri within 60 days of hire and BLS - Basic Life Support within 90 days of hire are required.

Responsibilities

• Evaluates patients referred by providers.
• Plans a treatment program, using appropriate treatment procedures that will best accomplish the established goals.
• Implements the treatment program using approved procedures.
• Documents treatment procedures and the patient's response to them, on a regular basis.
• Responsible for treatment procedures, documentation, and supervision of physical therapy assistants, physical therapy students, and Rehab Aides.
