The position involves facilitating the restoration of function and prevention of disability for patients with rehabilitation needs and/or physical disabilities by planning and administering medically prescribed physical therapy. The role requires a comprehensive approach to patient care, including assessment, planning, implementation, and evaluation of rehabilitative care, as well as providing information and support to patients and their families.

  • Identifies patient rehabilitation care requirements by assessing patient condition upon order of physician and at regular intervals during assigned shift.
  • Plans, implements, and evaluates rehabilitative care by setting goals, collaborating with the healthcare team, and documenting interventions and responses.
  • Provides information to patients and healthcare team by meeting them and answering questions related to treatments and continuing care requirements.
  • Monitors delivery of care by completing patient rounds, documenting care, and maintaining ongoing communication with care management.
  • Assures quality of care by measuring outcomes against patient care goals and adhering to applicable regulations and standards.
  • Protects self, co-workers, and patients by implementing policies to prevent the spread of diseases and adhering to infection-control protocols.
  • Respects patients by recognizing their rights and providing emotional, psycho-social, and spiritual support.
  • Maintains a safe and clean working environment by complying with procedures and keeping therapy areas orderly.
  • Ensures operation of equipment by following manufacturers' instructions and maintaining equipment inventories.
  • Maintains supplies inventory by checking stock levels and placing orders as needed.
  • Maintains professional and technical knowledge by attending workshops and participating in professional societies.
  • Contributes to the team effort by following through on delegated tasks and assisting coworkers as needed.
  • Promotes the development of Good Shepherd as a system by volunteering at community events and acting as a preceptor for new associates.
  • Bachelor's Degree in Physical Therapy required.
  • Previous acute care experience or similar setting required.
  • Physical Therapist license required.
  • Basic Life Support / CPR certification as a healthcare provider per the American Heart Association required.
