About The Position

As a Home Health Physical Therapist, you'll provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury. You will work as part of a home health team which may include an RN, LPN, CNA, HHA, OT, and SLP, focused on the patient's plan of care and goals. Your role will involve observing movements, conducting assessments, documenting progress, and modifying care plans as needed. You will assist patients with exercises and hands-on therapy to manage pain and increase mobility, educate patients on their plan of care, and coach families on safely assisting with exercises.

Requirements

  • Current license or certification as a physical therapist in the state of Maryland.
  • A graduate of an accredited physical therapy education program recognized by the APTA.
  • Two years of experience as a physical therapist relevant to the patient population assigned in the last five years.
  • Covid-19 vaccination card.
  • Valid driver's license.
  • Valid I-9 documentation.
  • Current TB test or X-ray.
  • Pass federal and state required criminal and abuse background checks.

Responsibilities

  • Assist the physician or other licensed practitioner in evaluating the level of patient function and performance and equipment needs.
  • Develop the resulting plan of care/treatment including appropriate long- and short-term treatment goals.
  • Complete initial and ongoing comprehensive assessments at appropriate time points.
  • Provide interventions and therapeutic treatment in accordance with the physician certified plan of care.
  • Regularly assess changes in the patient's status and determine whether the planned services should be modified.
  • Advise and consult with the family and/or other caregivers to promote patient progress.
  • Participate in educating the patient and the family to promote patient progress.
  • Actively communicate with other members of the healthcare team.
  • Prepare and submit documentation of treatment and skilled intervention provided.
  • Participate in quality and performance improvement measures.
  • Participate in in-services or training as requested.
  • Supervise professional and paraprofessional staff as assigned.
  • Read and interpret technical instructions related to patient care.
  • Visually and aurally observe and assess the patient.
  • Perform and prioritize multiple functions or tasks.
  • Provide proof of valid driver's license and auto liability insurance if required.
  • Travel within the geographic area serviced by the home care.
  • Complete other assignments as requested.
  • Meet health requirements to provide patient care.
  • Access, use, and disclose personal health information as necessary.
