About The Position

The position involves providing services that identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain. The role requires evaluating and analyzing patients, applying therapeutic techniques, utilizing physical and chemical agents, and instructing patients and their families. The professional will assume responsibility for ongoing interdisciplinary assessments and the development of individualized care plans in collaboration with patients, their representatives, and caregivers. Effective communication with the responsible physician and other healthcare practitioners regarding the current home health plan of care is essential.

Requirements

  • Bachelor's degree in Physical Therapy (PT)
  • Minimum of one year of experience in a related field.

Responsibilities

  • Performs physical therapy assessment including functional limitations, mobility status, endurance status, and ADL status.
  • Identifies patient and family needs for other home health services and refers for same when needed.
  • Provides services that are ordered by the physician as indicated in the plan of care.
  • Communicates with the physician(s) involved in the plan of care and other health care providers as appropriate.
  • Provides appropriate treatment to relieve pain, develop and restore function, and regain optimum level of independence using appropriate equipment and modalities.
  • Records all evaluation data, treatments, and client's response to therapy on a visit basis, and provides client's discharge planning.
  • Participates in case management conferences with team members to coordinate physical therapy and total plan of care.
  • Provides counseling and education to patients, family, and other health care personnel in the patient’s treatment regimen, and documents to whom this instruction has been given.
  • Provides or arranges for equipment required to increase the client's function and independence.
  • Participates in the agency quality assurance and performance improvement program and the agency-sponsored inservice training.
  • Supervises care provided by the PTA(s) and home health aides as appropriate.
  • Fulfills all compliance responsibilities related to the position.
  • Performs other duties as assigned.
