About The Position

Napa Post Acute is seeking Physical Therapists (PTs) to join our rehab team. We are looking for caring and motivated therapists who are passionate about helping residents regain independence and improve their quality of life.

Requirements

• Current California Physical Therapist license
• Strong communication and teamwork skills
• Commitment to high-quality, patient-centered care
• Skilled nursing or post-acute experience preferred (new grads welcome)

Responsibilities

• Provide physical therapy services to residents in a skilled nursing setting
• Develop and implement individualized treatment plans
• Document care and progress accurately and in compliance with standards
• Work closely with nursing staff, physicians, and interdisciplinary team members
• Help residents achieve mobility, strength, and independence goals
