Hartford HealthCare Rehabilitation Network is seeking a Physical Therapist for our newest clinic located in Norwalk, CT. This clinic is part of a multidisciplinary team of medical, rehabilitation, and wellness professionals dedicated to improving the health of the community. This clinic includes specialty programs in: Pelvic Health, Lymphedema, Orthopedic Physical Therapy, Orthopedic and Neurologic Occupational Therapy, Back to Life Fitness program This position will primarily treat Ortho/Sports Med populations. Requirements

  • Graduate of Physical Therapy Program from an accredited American Physical Therapy Association school, with a BS, master's, or doctorate degree
  • CT active or temporary license
  • Will consider new graduates with an interest in ortho/sports med

Nice To Haves

  • Interest in ortho/sports or becoming trained in this area is preferred.

Responsibilities

  • Conduct comprehensive evaluations to understand each patient's unique needs-strength, mobility, balance, endurance, and more.
  • Design individualized, patient-centered care strategies that promote optimal movement, safety, and long-term wellness.
  • Implement and delegate treatments using advanced interventions, including therapeutic exercise, mobilization, and modalities such as heat, cold, water, and electrical stimulation.
  • Work closely with a multidisciplinary team-Rehab Therapists, Nurses, Social Workers-to ensure seamless care and smooth transitions, including discharge planning and community resource recommendations.
