You provide skilled care to the patient as prescribed by the physician and in compliance with regulations as established by the Indiana State Board of Nursing and policies of the Agency.

Requirements

Bachelor of Science or advanced degree in Physical Therapy
Current state license/certification
Current registration with the American Physical Therapy Certification Board
Excellent oral, written communication and interpersonal skills
Demonstrate competency in applying the principles, methods, materials and equipment used in specific therapy discipline
Knowledge of accreditation standards and compliance requirements

Nice To Haves

Prefer minimum one (1) year experience in rehabilitation

Responsibilities

responsible for the physical care of the patients in their homes
provide skilled care to the patient as prescribed by the physician and in compliance with regulations as established by the Indiana State Board of Nursing and policies of the Agency
evaluate and treat patients, in accordance with the orders from the physician, through the use of a variety of therapeutic procedures designed to restore loss of muscle strength, coordination, body alignment, to relieve pain, improve movement and function
work as a member of a team providing support and information within the area of Physical Therapy
provide therapy services in locations including, but not limited to, Skilled Nursing Facilities, Outpatient Facilities, Long Term Care Facilities or Home Health Care
