Requirements

• Kentucky license as a physical therapist required
• Requires a Bachelor, Master or Doctoral degree from a physical therapy program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE), or higher
• BLS preferred at hire, required within 90 days after hire

Responsibilities

• Performs all patient evaluations
• Writes Plan of Care in accordance with MD
• Completes required documentation and submits within the allotted timeframe
• Maintain standards of quality in accordance with accepted standards and hospital and departmental policies and procedures
