About The Position

The Physical Therapist provides medically prescribed physical therapy evaluation and treatment to restore function, improve mobility, and prevent disability in patients with musculoskeletal, orthopedic, cardiopulmonary, neurological, and integumentary conditions. Services are delivered primarily in Inpatient Rehabilitation, with support in Acute Care and Outpatient Rehab as needed, in compliance with hospital policy, state licensure, and federal regulatory requirements. Lake Huron Medical Center is a 139-bed community hospital that provides a wide range of inpatient and outpatient health care services to residents of St. Clair and Sanilac counties including emergency services, primary stroke center, cancer care, surgical services, physical therapy and rehabilitation services, pulmonary rehabilitation, and specialized diabetes programs. Requirements

  • Bachelor/ Masters/ Doctorate/Certificate degree in Physical Therapy.
  • Current state Physical Therapy license.
  • Current BCLS (AHA) certificate upon hire and maintain current.

Responsibilities

  • Perform physical therapy evaluations and reassessments
  • Develop and implement individualized plans of care per physician orders
  • Provide skilled physical therapy treatment, including therapeutic exercise, gait training, balance training, neuromuscular re-education, manual therapy, and functional mobility
  • Educate patients and caregivers on treatment goals, home exercise programs, and safety
  • Document evaluations, treatments, and discharges in the electronic medical record (EMR)
  • Collaborate with interdisciplinary healthcare teams
  • Maintain compliance with state practice acts, CMS, Joint Commission, and hospital standards
  • Participate in weekend rotation coverage
