Requirements

Current state licensure as a Physical Therapist (PT)
Graduate of an accredited Physical Therapy program
Compassion, adaptability, and a passion for serving seniors

Nice To Haves

Home health experience preferred, but not required

Responsibilities

Evaluate and develop individualized treatment plans to restore function, improve mobility, and relieve pain
Provide direct therapy services in the home setting, helping residents achieve their highest level of independence
Collaborate with the interdisciplinary team to ensure coordinated and comprehensive care
Document patient progress and outcomes accurately and timely
Educate patients, families, and caregivers on therapy techniques and home exercise programs
