About The Position
Are you a passionate Physical Therapist looking for a rewarding career in home health? Join UVM Health’s Adult Home Care team and help patients maintain independence, improve mobility, and enhance overall well-being—all in the comfort of their own homes . We are looking for therapists that are available weekdays. As a home health physical therapist, you’ll be part of a collaborative interdisciplinary team including RNs, LPNs, LNAs, Occupational Therapists, Speech-Language Pathologists, Social Workers, and administrative staff 🤝, providing comprehensive, patient-centered care. As an Adult Home Care Physical Therapist, you will: Perform comprehensive evaluations and assessments 🩺 Develop individualized treatment plans tailored to each patient’s needs ✍️ Deliver skilled therapy interventions to improve mobility, strength, and function Support patients in remaining in their homes safely and independently Our coverage area is segmented into four territories within Chittenden and Grand Isle Counties. Case load territory is assigned after orientation. We offer services for all ages and stages of life, including home health nursing, physical, occupational and speech therapy, children’s and family services, as well as palliative and interdisciplinary hospice care.
Requirements
- Bachelor’s degree or higher in Physical Therapy
- Current Vermont Physical Therapist license, or willingness to obtain one
Nice To Haves
- 2+ years of general PT experience preferred, or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance
- Home health experience preferred, but not required, we can train the right person!
