About The Position

FOX Rehabilitation is seeking passionate, licensed Physical Therapists in Tomball, TX, to deliver exceptional one-on-one care to older adults. This role involves providing therapy in private residences and Senior Living Communities, utilizing the signature Geriatric House Calls™ model to improve patients' mobility, independence, and quality of life. Physical Therapists will enjoy autonomy and flexibility to create and manage their own treatment schedules, fostering strong, lasting patient relationships. The opportunity also allows for the incorporation of telehealth and other forms of virtual care to enhance flexibility and continuity of care. Requirements

  • Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
  • Degree from an accredited Physical Therapy program
  • Basic computer and EMR proficiency
  • Current CPR certification
  • Ability to meet the physical demands of in-home and community-based care, including: Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
  • Ability to meet the physical demands of in-home and community-based care, including: Performing CPR
  • Ability to meet the physical demands of in-home and community-based care, including: Providing moderate to maximum assistance with patient transfers and bed mobility
  • Ability to meet the physical demands of in-home and community-based care, including: Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
  • Ability to meet the physical demands of in-home and community-based care, including: Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
  • Ability to meet the physical demands of in-home and community-based care, including: Wearing PPE when required

Responsibilities

  • Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
  • Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
  • Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
  • Document effectively within FOX’s EMR system in accordance with regulatory standards
  • Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
  • Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
  • Provide telehealth / virtual care when appropriate to extend access and flexibility
