About The Position

Sunnyvale Gardens Post Acute is seeking a PRN Physical Therapist to join their skilled nursing and post-acute care team. Requirements

  • Current California Physical Therapist license in good standing
  • Graduate of an accredited Physical Therapy program
  • Strong communication, clinical, and documentation skills
  • Ability to work independently and as part of a team

Nice To Haves

  • Experience in skilled nursing or post-acute care preferred

Responsibilities

  • Evaluate patients and develop individualized treatment plans
  • Provide therapeutic exercise, gait training, balance training, and functional mobility interventions
  • Document evaluations, treatments, goals, and progress in accordance with regulatory standards
  • Collaborate with an interdisciplinary care team to optimize patient outcomes
  • Educate patients and families on treatment plans, safety, and home exercise programs
  • Maintain compliance with state, federal, and facility policies
