Requirements

  • Unencumbered Florida Physical Therapy License (required)
  • Valid driver’s license, auto insurance, and reliable transportation (required)
  • CPR Certification (required)

Nice To Haves

  • Experience with OASIS documentation (preferred)
  • Ability to delegate and supervise Physical Therapy Assistants (preferred)

Responsibilities

  • Travel to patients’ homes to provide individualized physical therapy services that promote recovery, mobility, and independence
  • Conduct comprehensive evaluations to assess patient conditions and rehabilitation needs
  • Develop and implement goal-oriented treatment plans in collaboration with patients, caregivers, and the interdisciplinary care team
  • Utilize AI-assisted documentation and charting tools to support timely, accurate note completion and faster turnaround times
  • Guide and supervise Physical Therapy Assistants to ensure high-quality, compliant care delivery
