What You’ll Do:
Deliver exceptional PT services to evaluate and treat patients in their homes Work closely with a compassionate care team Help patients regain independence and quality of life

Requirements

• Active CO PT license
• 1+ year in acute care or home health (required)
• Passion for patient-centered care
• EMR experience

Nice To Haves

• Medicare home health experience is highly preferred

Responsibilities

• Evaluate and treat patients in the home
• Help improve mobility, strength, and safety
• Document using EMR (Kantime a plus)
