About The Position

"Navitas Healthcare, LLC" is seeking a skilled Physical Therapist to provide high-quality, patient-centered rehabilitation services primarily in an outpatient setting, with occasional inpatient acute and swing-bed rehabilitation coverage. This role focuses on evaluating, treating, and educating patients with orthopedic, neurological, balance, and general rehabilitation needs while maintaining a manageable caseload that supports quality care.

Requirements

  • Graduate of an accredited Physical Therapy program.
  • Current Physical Therapist licensure in the practicing state.
  • Strong clinical assessment, documentation, and patient communication skills.
  • Ability to work independently and manage time effectively in a mixed-setting environment.

Nice To Haves

  • Experience in outpatient orthopedics preferred.
  • Experience with inpatient acute care or swing-bed rehabilitation settings is a plus.

Responsibilities

  • Evaluate patients and develop individualized treatment plans based on clinical findings and functional goals.
  • Provide outpatient physical therapy services for general orthopedic conditions, including shoulder, back, hip, and knee diagnoses.
  • Treat patients with balance, fall-risk, and neurological conditions using evidence-based interventions.
  • Deliver inpatient physical therapy services as needed for acute and swing-bed rehabilitation patients.
  • Manage a caseload of approximately 15 patient sessions per day while adhering to scheduled appointment times.
  • Conduct patient evaluations lasting approximately one hour (4–5 per day) and follow-up treatment sessions of approximately 30 minutes each.
  • Ensure safe and effective treatment practices without routine double-booking or over-scheduling.
  • Document evaluations, treatment sessions, progress notes, and discharge plans in accordance with facility and regulatory standards.
  • Educate patients and caregivers on treatment plans, home exercise programs, and injury prevention strategies.
  • Collaborate with physicians, nursing staff, and interdisciplinary team members to support optimal patient outcomes.
  • Adhere to facility policies, professional standards, and patient safety guidelines.
