Concordia at Bethlen is a faith-based, non-profit continuing care retirement community located in historic Ligonier, Pennsylvania. The service organization consists of 44 retirement living cottages, 72 personal care units and 96 skilled nursing units. Located in Ligonier, PA, Concordia at Bethlen is seeking a Physical Therapist. The Physical Therapist is a licensed professional responsible for evaluating, planning, developing and implementing physical therapy to the residents of Concordia at Bethlen Skilled Nursing. Requirements

  • Graduate of a state-approved School of Physical Therapy accredited by the American Physical Therapy Association
  • Licensed as a Physical Therapist in the Commonwealth of Pennsylvania
  • CPR certification strongly preferred and encouraged

Nice To Haves

  • Two years of long-term care or rehabilitation center experience preferred.

Responsibilities

  • Performing comprehensive evaluations of residents.
  • Developing plans of care.
  • Providing therapeutic actions to relieve pain and maximize independence.
  • Instructing residents, families, and caregivers in the total physical therapy program.
  • Planning safe discharges with the resident, family, caregivers, and physician.
  • Supervising and delegating Physical Therapy Assistant(s) and Student Physical Therapists.
  • Completing timely documentation.
  • Committing to and maintaining high ethical standards and complies with the Code of Conduct.
