Requirements

  • Current, valid Physical Therapist (PT) License in the state of California.
  • A passion for geriatric rehabilitation and a team-first attitude.

Responsibilities

  • Clinical Excellence: Develop and implement evidence-based treatment plans tailored to short-term rehab and long-term care residents.
  • Rehab Leadership: Supervise Physical Therapy Assistants (PTAs) and collaborate with the nursing department to train Restorative Aides.
  • Interdisciplinary Teamwork: Actively participate in Patient Care Conferences, Utilization Review, and Rehabilitation meetings to ensure a holistic approach to recovery.
  • Precision Documentation: Complete evaluations, daily notes, and weekly progress reports in accordance with California PT Board standards and facility policies.
  • Discharge Mastery: Lead the discharge planning process, including family education, caregiver training, and recommending necessary durable medical equipment (DME).
  • Advocacy: Communicate effectively with physicians and supervisors regarding resident progress, ensuring all treatment plans are optimized for success.
