About The Position

We are seeking a Physical Therapist to provide skilled physical therapy services to patients across Senior Living Communities . In this role, you will be instrumental in helping seniors recover, enhance mobility, and maintain their independence through tailored physical therapy treatment plans. Requirements

  • Valid Physical Therapist license in Arizona , in good standing.
  • Two years of Home Health experience as a Physical Therapist preferred.
  • Possesses and maintains current CPR certification.
  • Licensed driver with reliable vehicle and current vehicle insurance.
  • Ability to work independently and manage time efficiently.
  • Excellent observation, verbal and written communication skills, problem solving skills, and organizational skills.
  • Excellent communication and interpersonal skills to work effectively with residents, families, and the healthcare team.

Nice To Haves

  • Experience in geriatric physical therapy or working with seniors is preferred.

Responsibilities

  • Evaluate physician’s referral and patient’s medical record.
  • Assessing and evaluating therapeutic/rehabilitative/functional status using approved evaluation techniques, tests and observations, and participation in the development of the total plan of care.
  • Evaluates the home environment.
  • Determines priority needs for physical therapy.
  • Directs physical therapy treatment.
  • Reports to physician, patient’s response to treatment and changes in condition.
  • Identifies patient and family/caregiver needs for other home health services and refers as necessary.
  • May train patients in the use of prosthetic device.
  • Administer a prescribed physical therapy program to restore and maintain maximum function and prevent disability using a range of therapeutic modalities.
  • Maintain appropriate documentation in the patient chart, and re‐evaluate patient progress and revise treatment programs when necessary.
  • Communicates physical therapy philosophy and treatment approaches and goals to patients, families and other members of the rehabilitation team.
  • Supervise Physical Therapy Assistants according to Agency policy and state regulations.
