About The Position

Join Transitions At Home, a not-for-profit home health organization, and help patients recover and thrive from the comfort of their homes. We’re hiring for a Part-Time (20–30 hrs/week) or Full-Time (30+ hrs/week) Physical Therapist to support our growing team in Southwest Wisconsin. Why Join Us? Requirements

• Active Wisconsin Physical Therapist license
• CPR certification
• Valid WI driver’s license & car insurance
• Two (2) years of experience preferred
• Comfortable working independently in a home care environment

Responsibilities

• Complete OASIS assessments, evaluations, and progress notes
• Develop and revise care plans
• Perform skilled physical therapy procedures
• Educate patients and families on home programs and daily activities
• Collaborate with physicians and the care team
• Participate in IDT meetings, in-service training, and QA initiatives
• Ensure accurate documentation and use of ICD-10 codes
