Concordia Home Health and Hospice of Bethlen is hiring a Licensed Physical Therapist PT to join its growing home health team in Johnstown and surrounding areas!! Requirements

  • A graduate of an accredited Physical Therapy program with a valid state license.
  • A passion for working with patients and a commitment to delivering high-quality, patient-centered care.
  • Exceptional communication and interpersonal skills, with the ability to connect with diverse patient populations.

Responsibilities

  • Conduct comprehensive evaluations and develop individualized treatment plans that empower patients to reach their maximum potential.
  • Implement evidence-based therapeutic interventions, including manual therapy, therapeutic exercises, and modalities.
  • Monitor patient progress, adjust treatment plans as needed, and ensure timely and thorough documentation.
  • Educate patients and their families on treatment options, rehabilitation techniques, and preventative measures.
  • Collaborate with physicians and other healthcare professionals to ensure a holistic approach to patient care.
  • Help create a welcoming and supportive clinic environment where patients feel comfortable and motivated
