About The Position

Our client provides comprehensive, team-based care for seniors (55+) through the PACE model (Program of All-Inclusive Care for the Elderly), offering medical, dental, therapy, transportation, meals, and personal care, often at no cost for those with Medicare/Medi-Cal, focusing on individualized plans and improving quality of life in the community. The Physical Therapist is a key member of the team, focusing on care planning and the coordination and delivery of restorative and rehabilitative physical therapy services to participants, working to maximize independence and safety. This role is different because Lower patient volumes – no more seeing 3-4 patients an hour Form long-term relationships with participants rather than short-term rehab stints Collaborate with other therapists, providers, and disciplines Have work/life balance - no hours of documentation after work hours On the day-to-day, you will: Conduct Physical Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals Actively participate in IDT meetings to develop participant care plans Deliver and document interventions as agreed upon in participant care plans Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP) Provide training and instruction to staff, participants, and caregivers/families on the use and care of therapeutic equipment

Requirements

  • Bachelor’s degree in physical therapy; master’s or doctoral degree preferred
  • Two (2) years in clinical setting with frail or elderly population preferred
  • Valid California Physical Therapy License, or proof of license eligibility
  • Current CPR certification preferred

Responsibilities
