About The Position

Physical Therapist (PT) – PRN Facility: Gateway Post Acute Location: Porterville, CA Employment Type: PRN / Per Diem Pay Range: $60.00 – $65.00 per hour (BOE) Setting: Skilled Nursing Facility | Short-Term Rehabilitation Join a Team That Puts People First—Starting With You Gateway Post Acute is seeking a compassionate and motivated Physical Therapist (PT) to join our rehabilitation team on a PRN basis. Our Porterville facility has proudly served the community for over 40 years, delivering high-quality, resident-centered care in a supportive and collaborative environment. General Purpose The Physical Therapist evaluates and treats residents, develops and implements individualized treatment plans, collaborates with physicians and interdisciplinary team members, and maintains accurate and timely documentation in the medical record. The PT also provides education and clinical guidance to staff to support optimal resident outcomes.

Requirements

  • Bachelor’s degree in Physical Therapy required
  • Current, active California Physical Therapist license in good standing
  • Must meet all continuing education requirements per state regulations
  • Strong clinical judgment and problem-solving abilities
  • Excellent communication and interpersonal skills
  • Proficiency with electronic documentation systems (PointClickCare and/or Casamba preferred)
  • Ability to work independently and collaboratively within a team
  • Frequent standing, walking, pushing, pulling, and patient handling
  • Ability to lift and move up to 50 pounds
  • Visual and auditory acuity sufficient for patient care and documentation

Nice To Haves

  • Master’s or Doctorate degree preferred
  • Skilled Nursing Facility or geriatric experience preferred but not required

Responsibilities

  • Evaluate residents’ physical conditions and functional abilities
  • Develop individualized treatment plans and obtain physician approval
  • Provide therapeutic interventions including therapeutic exercise, gait training, balance training, and mobility techniques
  • Monitor resident progress and modify treatment plans as appropriate
  • Document evaluations, daily treatment notes, progress notes, and discharge summaries in accordance with regulatory and payer requirements
  • Educate residents and families on mobility, safety, and injury prevention
  • Collaborate with nursing and interdisciplinary team members to ensure continuity of care
  • Participate in discharge planning as needed
  • Conduct in-services and training sessions for facility staff as assigned
  • Maintain compliance with all federal, state, and facility policies and procedures
  • Report equipment issues to ensure safe and effective operation
  • May supervise Physical Therapy Assistants (PTAs), aides, and students in accordance with state practice act
