About The Position

Ohana Pacific Health is Hawaii's largest, locally owned post-acute healthcare company, dedicated to transforming healthcare throughout Hawaii with an organizational culture based on excellence and genuine care. The company is seeking a passionate Physical Therapist (PT) to develop and implement progressive rehabilitation programs for patients in their homes. This role involves working in accordance with physical therapy principles and client needs, serving as a case manager, and supervising home health aides and physical therapist assistants when appropriate.

Requirements

Master’s Degree or Doctorate Degree in Physical Therapy
Have successfully completed and graduated from an accredited occupational therapy professional program recognized by NBCOT and have completed all fieldwork requirements
Current medical/health screening including 2 step TB clearance.
Valid State of Hawaii license to practice as an Physical Therapist.
Current Basic Life Support CPR (cardiopulmonary resuscitation) certification through the American Heart Association
Two years of experience as an physical therapist, preferably in home health or other healthcare practice setting.

Responsibilities

Evaluates assigned patients. Admits those who meet agency and regulatory criteria.
Plans, coordinates, and implements the appropriate treatment according to patient’s needs. Performs ongoing reassessments. Discharges patients when goals are met, or patient does not meet home health requirements.
Develop Home Health Aide (HHA) and Physical Therapy Assistant (PTA) care plans. Delegates duties to HHA’s and PTAs (Physical Therapy Assistant) based on agency and regulatory policies. Supervises and provides input to HHA's and PTAs per agency policy.
Assumes responsibility for case management of assigned patients. Consults with appropriate community and internal resources and makes appropriate interdisciplinary referrals.
Performs patient care duties and/or treatments based on specific needs, including medical condition, age, and/or developmental status.
