Essential Job Duties: (These statements are intended to describe the essential functions and related requirements of persons assigned to this job. They are not intended as an exhaustive list of all job duties, responsibilities and requirements.) 1. Performs all essential job duties of staff Physical Therapist (see below). 1. Evaluates or reassesses patient progress towards established goals. Performs patient evaluations with utilization of standardized assessments and clinical judgement to determine patient problems to address 2. Implements or supervises (per state practice act) patient treatments within the established plan of care 3. Communicates and reports current patient progress and discharge planning as appropriate with referring provider 4. Documents within state practice act guidelines and ensures that documentation meets requirements of organization and payor sources 2. Performs patient evaluations to determine patient deficits, need for therapy, and established client-centered goals based upon identified problems to address. Utilizes evidence-based practice and standardized assessment tools to support evaluation findings objectively. 3. Discharge planning including home assessments and community outings as beneficial/appropriate, determination of a/e needs, communication to the patient, family, and other healthcare professionals through education or team conference meetings if applicable  4. Develops an individualized patient plan of care, physical therapy interventions, and expected outcomes based on the evaluation and documents findings. 5. Exercises the skills, safeguards, and professional conduct of a Physical Therapist. 6. Supervises PT/PTA clinical students, student volunteers, and shadow students 7. Supervises PTAs and rehab technicians on rotation 1. Present and available as a resource for other physical therapy staff 2. Professional in method of communication and strives to facilitate team rapport through interactions 3. Follows state practice act guidelines in supervision of PTAs and technicians 4. Co-signing within scope of practice and state guidelines, supervising, and ensuring availability as a resource for other clinicians   5. Monitors and updates both patient POC and established goals within state practice act and organizational timelines to ensure optimal provision of care from PTAs 8. Demonstrates knowledge and clinical competence within personal area of clinical care, possibly including but not limited to: 1. Performs home assessments as coordinated and requested, maintaining compliance with departmental policy 2. Participates and reports in weekly patient conference with current patient progress and discharge planning with team staff members 3. Orthotic fitting, training, and management, following organizational protocols to ensure reimbursement 4. CMS and payor guidelines and requirements specific to inpatient acute, inpatient rehab, and outpatient therapy settings 5. Contributes to a high-performance work environment by seeking additional resources that enhance and maintain current knowledge and skills 9. Demonstrates thorough and comprehensible documentation that supports clinical provision of care and optimal reimbursement. Documents patient response to treatment as well as thorough evaluations, progress notes, and discharge summaries within the designated time frames. 10. Completes other duties and special projects as assigned from Manager or Director   Secondary Job Duties That May be Reassigned: 1. Participates within departmental expectation of rotations per Physical Medicine and Rehab Therapy policy, which may include a holiday and/or weekend rotation. 2. Responsible for reporting breakdown of necessary support services and providing suggestions for improvement in services to Manager and/or Senior Therapist. 3. Assists in orienting/training staff members. 4. Assists with departmental Performance Improvement Program. 5.
