About The Position

The Physical Therapist is responsible for assessing and identifying patient care needs and priorities for care through evaluation, assessment, planning, providing treatment and delegating care for patients with physical and/or cognitive disabilities to improve functional independence. This process focuses on patient/family needs and assessing the patient’s response to care and progress toward outcomes. Complies with department and organization-wide policies, Critical Access Hospital requirements, Safety, and Infection Control, and all Federal and State of Colorado regulations related to the performance of Keefe Memorial Hospital operations and requirements of the compliance program.

  • Maintains confidentiality of patient information.
  • Exercises professional judgment, courtesy, and professionalism in daily interactions with others.
  • Establishes and maintains effective working relationships with health care team members and diverse patient populations.
  • Utilizes proper body mechanics, transfer/lifting techniques and appropriate equipment to minimize fall risk to patient and injury to self.
  • Bachelors in PT required
  • Must be familiar with the principals and practices of Physical Therapy according to the APTA Code of Ethics as outlined by the State of Colorado Health and Human Services
  • Current Colorado State License
  • Current Basic Life Support (BLS)

  • Masters or Doctorate degree preferred

Responsibilities

  • Performs and documents initial patient evaluation and assesses data to identify problems.
  • Establishes, documents, and provides a plan of care with functional, measurable goals based on evaluation of the examination data and patient needs.
  • Provides or delegates and supervises physical therapy patient interventions in a safe, predictable manner consistent with the established data evaluation and the plan of care.
  • Provides and documents patient/family education regarding current condition and plan of care, including home program/discharge instructions.
  • Performs and documents patient re-examination, re-evaluates the data, and modifies and documents the plan of care.
  • Communicates relevant patient care information with other health care professionals.
  • Participates in personal and professional development by attending and/or presenting inservices (required, voluntary), department staff meetings, seminars, community education programs and seeking ongoing developmental and educational opportunities.
  • Maintains current clinical competency per department guidelines.
  • Uses quality improvement processes, programs, or outcomes to help improve department operations.
  • Performs other duties as assigned.
