About The Position

The Physical Therapist (PT) is responsible for evaluating and assessing the needs of referred patients and formulating treatment plans within a home care environment. The PT provides therapy services as defined in these treatment plans, working cooperatively with physicians and case managers. This role involves a variety of tasks and encourages the use of creativity and latitude.

Requirements

• Graduate of a Baccalaureate, Master's or Doctorate program in Physical Therapy program accredited by a Physical Therapy accreditation
• Current and unencumbered licensed to practice as a Physical Therapist within the state of employment
• Two (2) years of appropriate experience as a Physical Therapist
• Demonstrates good verbal and written communication and organization
• Possess and maintain valid CPR obtained in-person with a practical skills evaluation as part of the final test
• Access to reliable transportation

Nice To Haves

• Community/home health experience is a plus
