Requirements

  • Graduate of an accredited school with a minimum of a bachelor’s degree in Physical Therapy.
  • Certification or license as required by the State of Wisconsin or applicable state to practice as a Physical Therapist.

Responsibilities

  • Perform initial and ongoing evaluations of residents’ physical abilities, mobility, strength, balance, and functional status
  • Identify impairments and barriers to safe and independent mobility
  • Develop and implement individualized, goal-driven physical therapy treatment plans
  • Address gait, balance, endurance, and functional mobility needs
  • Support recovery following injury, surgery, illness, or neurological conditions
  • Treat residents with chronic conditions such as arthritis, Parkinson’s disease, osteoporosis, or post-stroke deficits
  • Improve performance in functional activities including transfers, walking, stair negotiation, and fall recovery
  • Provide therapeutic exercise, neuromuscular re-education, and manual therapy as appropriate
  • Recommend, fit, and educate residents on assistive devices and durable medical equipment
  • Perform fall risk assessments and implement fall prevention strategies
  • Educate residents, families, and staff on safe mobility techniques and home safety
  • Collaborate with interdisciplinary teams including OT, SLP, nursing, and care staff
  • Participate in care planning meetings to support holistic resident outcomes
  • Maintain accurate, timely documentation and ensure regulatory compliance
