Requirements

Current license as a Physical Therapist in the state of Michigan.
Bachelor’s or Master’s degree in Physical Therapy or related field.
Proven experience in home health care or clinical physical therapy preferred.
Strong interpersonal and communication skills.
Ability to work independently and manage time effectively in a home care setting.
Valid driver’s license and reliable transportation.

Responsibilities

Assess patients’ physical condition and develop individualized treatment plans.
Deliver hands-on therapeutic interventions and exercises to improve mobility and function.
Monitor patient progress and adjust treatments accordingly.
Collaborate with physicians, nurses, and other healthcare professionals to coordinate care.
Maintain accurate and up-to-date patient records and documentation.
Educate patients and their families on exercises and strategies for recovery and prevention.
