About The Position
We're hiring a Physical Therapist for our Hospital At Home Department in Jacksonville, Florida. This is a Full-Time Days position which will provide patient home-based healthcare throughout the Jacksonville community. The schedule for this position will be Monday - Friday with no weekend or holiday commitments. We are looking for a Physical Therapist to join our Rehab Services Team at Baptist Medical Center. This position requires a candidate with at least 1 year of recent experience working in an acute care setting, home health patient care experience, and experience providing direct orthopedic and neurological physical care. Requirements
- Active BLS from the American Heart Association and licensure from the FL Department of Health required
- Bachelor's Degree
- 1 or more years of Physical Therapy Experience
- Experience working in an Acute Care setting preferred
- Experience working in a Home Health setting preferred
- Experience with Surgical Patient Care
- Experience in Orthopedics
- Experience in Neuro

Responsibilities
- Evaluates, plans, directs and administers physical therapy treatment to patients.
- Provides patient care to help improve or restore function and prevent disability following injury, disease or physical disability.
- Oversees support staff.
- Participates in departmental operations and in performance improvement activities.
- Responsible for remaining within budget guidelines.
- Participates in mandatory education and licensure requirements.
