About The Position

The Physical Therapist provides specialized physical therapy services within the home health and hospice environment, delivering comprehensive, patient-centered care while ensuring accuracy, safety, and compliance with all regulatory standards. This role provides rehabilitative services to home health and hospice patients, carrying out skills that support treatment, rehabilitation, safety, maintenance, and education for patients and their families, with particular attention to the geriatric population. Working as part of a collaborative interdisciplinary care team, the Physical Therapist plans and coordinates therapy services for individuals and families, assesses health needs and potential or existing hazards, and consults with or refers patients and families to appropriate agency or community resources as needed. Treatment and preventive procedures are delivered under medical direction, and care is evaluated, planned, implemented, supervised, and documented under the guidance of the supervisor or manager, in accordance with established standards of performance and organizational policies.

Requirements

  • Licensed Physical Therapist in North Dakota.
  • Current driver's license required and car available to travel to patient's home.
  • Organize time well and meet job expectations and performance standards.
  • Exhibits responsibility and flexibility in work situations, willing to work irregular and/or less favorable hours.
  • Demonstrates tact, sensitivity, sound judgment, and professional attitude when relating with patients, families and colleagues.
  • Meet the physical and emotional demand of the position.
  • Demonstrate a caring and respectful attitude when providing patient care.

Nice To Haves

  • Strongly prefer 2 years previous experience as a Physical Therapist.

Responsibilities

  • Perform comprehensive physical therapy assessments and develop individualized, evidence‑based treatment plans.
  • Supervises and coordinates care with PTA's.
  • Provide therapeutic interventions focused on mobility, strength, pain reduction, and functional independence.
  • Collaborate with interdisciplinary team members to coordinate patient‑centered care and discharge planning.
  • Educate patients and caregivers on safe mobility techniques, equipment use, and home‑exercise programs.
  • Document all communications, evaluations, progress notes, and care plans accurately and promptly in the electronic record.
  • Participate in interdisciplinary team meetings to communicate patient status, update care plans, and collaborate on individualized interventions.
