Requirements
- Arizona-licensed Physical Therapist (PT)
- CPR certification (AHA-approved)
- At least 1 year of clinical experience (Home Health experience preferred)
- Organized, compassionate, and reliable
- Valid driver’s license, auto insurance , and reliable transportation
- Strong communication skills and clinical decision-making
Nice To Haves
- Home Health experience preferred
Responsibilities
- Conduct in-home PT evaluations and follow-up visits
- Design and implement individualized rehabilitation and treatment plans
- Improve patients’ strength, balance, mobility, and functional independence
- Educate patients and caregivers on safe movement and equipment use
- Document visits using WellSky Kinnser EMR
- Coordinate care with physicians, RNs, case managers, and the interdisciplinary team
