What You'll Do: As a Home Health Physical Therapist, you'll: Conduct initial patient assessments in the home setting Develop and implement individualized treatment plans Coordinate care with physicians and healthcare teams Promote mobility, independence, and healing through compassionate, hands-on care Use our proven case management model to guide patient outcomes and support families
Requirements
- Active Florida Physical Therapist license
- Minimum 2 years of post-graduate experience preferred
- Passion for patient-centered care and commitment to quality outcomes
Nice To Haves
- Orthopedic or home health experience is a plus
