About The Position

As a Home Health Physical Therapist, you’ll deliver one-on-one patient care while playing a critical leadership role in supervising and guiding Physical Therapist Assistants (PTAs). Your expertise ensures every patient receives safe, effective, and goal-driven care—every visit, every time.

Requirements

• Licensed Physical Therapist (PT)
• Strong leadership, communication, and clinical decision-making skills
• Passion for accountability, quality care, and team development
• Successful completion of accredited physical therapy program and state licensure exam.
• Valid Physical Therapy license
• CPR certification
• Valid Driver’s license
• Valid Auto Insurance

Nice To Haves

• Experience supervising PTAs (preferred, but not required)
• 1 year of home health experience preferred
• HomeCare HomeBase experience preferred

Responsibilities

• Perform comprehensive evaluations and establish clinical diagnoses
• Develop and implement individualized treatment plans focused on functional outcomes
• Directly supervise PTAs, ensuring treatments align with the plan of care and regulatory standards
• Provide ongoing guidance, mentorship, and clinical oversight to PTAs
• Conduct reassessments and supervisory visits to ensure patient progress and quality outcomes
• Maintain accurate, compliant documentation for both your care and PTA services
• Collaborate closely with patients, families, and the interdisciplinary team
• Ensure infection control and patient safety standards are consistently followed
