About The Position

Physical Therapists work across outpatient, home health, and senior living settings, collaborating with a supportive, interdisciplinary team (PT, OT, SLP, wellness). Requirements

• Licensed Physical Therapist in Minnesota
• Degree from an accredited PT program (DPT preferred)
• A patient-first mindset and solid clinical judgment

Nice To Haves

• Experience in SNF, rehab, or senior living
• PTA supervision experience
• A passion for working with older adults

Responsibilities

• Help older adults get stronger, move better, and live more independently
• Build personalized treatment plans—not cookie-cutter protocols
• Work across outpatient, home health, and senior living settings
• Collaborate with a supportive, interdisciplinary team (PT, OT, SLP, wellness)
• Mentor PTAs and grow your leadership skills
• Adjust care in real time based on patient progress (because outcomes matter)
