Physical Therapist (PT) Home Health Albuquerque, NM and surrounding areas Full-Time | W2 | Direct Hire You Deserve More Than Just a Caseload This isnt volume-driven care. Youll receive 1:1 mentorship, clinical autonomy, and a sustainable caseload designed to prevent burnout and encourage growth. Requirements

Active PT license in New Mexico (or eligibility)
Minimum 1 year of direct home health experience (strongly preferred)
Commitment to evidence-based, patient-first care
Strong communication and organizational skills
Reliable transportation

Responsibilities

Conduct in-home physical therapy assessments and create customized care plans
Help patients regain independence and mobility in a home setting
Communicate progress and collaborate with nurses, physicians, and other disciplines
Ensure documentation is timely, compliant, and defensible
Empower families with education and resources for long-term success
