About The Position
We are looking for a dynamic, motivated Physical Therapist to join our practice full time in Chicago IL 60647. We are looking for a Physical Therapist who has experience working with active adults ages 30-60, fantastic ability to connect with patients, and is willing to work alongside a multidisciplinary team to help others. Requirements
- Graduation from an accredited Physical Therapy program
- Physical Therapist license in IL
- 3 + years’ Experience working with 30-60 year old active patients preferred
Responsibilities
- Patient evaluation/exam – medical history, review pertinent reports/imaging, and physical exam
- Treatment plan formulation- Work with advanced medical technology equipment as well as traditional physical therapy to create optimal treatment plans for patients
- Physical Therapy- therapeutic exercises and modalities
- Patient education about the diagnosis, care plan options, lifestyle modifications, and exercises
- Delegating exercise progressions to rehab techs and oversee rehab floor
- Collaboration with the other healthcare professionals for comprehensive patient care
- Documentation/reports- timely and accurate
